Referrals

The Early Intervention Referral form is here.

 

Any child age birth to three years with concerns about development may be eligible for services.  These concerns may include:
*Difficulty speaking clearly or understanding language
*Behavior difficulties
*Diagnosed hearing and vision loss
*Difficulty with movement

Any child having a condition of birth such as:
*Premature birth
*Cerebral palsy
*Down syndrome
*Fetal Alcohol Syndrome
*Spinal bifida

Anyone who is concerend about a child's development may make a referral:
*Parents
*Nurses
*Grandparents
*Social workers
*Doctors
*Teachers
*Friends

Early Intervention Services may include one or more of the following:
*Speech-language therapy
*Physical therapy
*Occupational therapy
*Developmental therapy
*Audiology
*Assistive technology
*Transportation
*Medical diagnostic services
*Nursing services
*Nutrition services
*Parent Liaison services
*Social work services
*Service coordination
*Special instructions
